Referral Process...

We will discuss the family situation and an application form will be completed by the referrer and sent to us. A copy of the application form can be downloaded from here Referral Form.xls

This is an excel form which you can either edit and return to us via email or print out and fill out by hand; returning to us with a self addressed A4 envelope.

We will then contact the family and relevant professionals to speak to them personally about the programme and a follow this up with a letter of the programme details, the venue and what is required to bring to the Break.

During the programme we will invite the referrer to visit the family as this will give them an experience of how we work and enable them to be able to monitor the progress of their family. At the end of the Break we will invite the families to complete an evaluation and monitoring form (completed with their link worker) and we will offer a family feedback session with their consent after the Break when they have returned home.

Following the programme we have a follow up meeting with the referrers, the managers and funders to discuss the evaluation and the way forward.
